Directions to Use the State Model Meeting Notice Form
The State鈥檚 Model Meeting Notice Form is provided as a Word document, which includes form fields to enter typed information.听 The form is 鈥渓ock protected鈥 in order for the form fields to function properly.听 The form should be used when it is in 鈥榣ock鈥 mode.听 Unlocking the form during use may result in a modification to the form itself.听 It is recommended that each district password protect the form to prevent inadvertent form modifications.听 To do so, go to the "Tools" "Options" "Security" and follow the directions.听
To use this form in its current format, you must follow the directions below.
- Save the document on your school district鈥檚 letterhead. In order to do this, it is necessary that the meeting notice form appear as a Word document and in 鈥榰nlock鈥 mode.听 To unlock the form, go to 鈥淰iew鈥 鈥淭oolbars鈥 and click on 鈥淔orms鈥.听 This action will activate the 鈥楩orms鈥 toolbar.听 From the 鈥楩orms鈥 toolbar, click the 鈥淧rotect Form鈥 symbol (padlock). The form will then be in 鈥榰nlock鈥 mode. Go to 鈥榚dit鈥, 鈥榮elect all鈥 and 鈥榗opy鈥.听 Paste the highlighted form onto district letterhead.听 The form, now on district letterhead, must be locked in order to be functional.听 To lock the form, go back to the 鈥淔orms鈥 toolbar and click the 鈥淧rotect Form鈥 symbol (padlock). The form will then be locked and ready for use.听 Save the document.
- To complete the form for each school-age student, identify the type of Committee meeting (i.e., CSE or Subcommittee) by using the 鈥榙rop-down鈥 field on the Committee on Special Education meeting notice.听
- Use the Tab button to advance from field (grey box) to field to insert the required information as appropriate.听These fields will automatically expand as text is entered.听 In the event that there is no relevant information to be included in a particular box, enter 鈥榥one鈥 or 鈥榥ot applicable.鈥櫶
- After entering all required information, SAVE the document to your file and PRINT.听 Although the fields display in grey while viewed on the screen, the grey will not show when the form is printed.听
